You have two options: print the blank template below and fill it out by hand, or fill it out on screen first for a cleaner PDF that both parties can read without guessing at handwriting. Both are legally equivalent when both parties sign and date.

Blank printable templates are useful when you're meeting the buyer in person, you don't have a printer where the signing will happen, or you just want a piece of paper to fill out with a pen. Print two copies — one for each party. Both sign, both keep one.

The trade-off: handwritten forms are easy to misread, easy to lose, and easy to challenge later when someone claims a number was "5" instead of "6". If you have five minutes and a device with a browser, typing it out on screen produces a cleaner document with no handwriting to argue about — and it costs nothing.
